Nasal stuffiness or obstruction can result from numerous conditions including inflammatory conditions such as allergies or sinusitis. However, often there is a structural abnormality in the nasal skeleton that is responsible for poor nasal breathing. The main structural abnormalities include a deviated nasal septum, weak or collapsed nasal cartilages, or asymmetric nasal bones. A septoplasty is a surgery that corrects the deviation by removing or straightening the deviated portions of this bone or cartilage. The external skeleton of the nose is comprised the paired upper lateral and lower lateral cartilages that provide support of the lower two-thirds of the nose and the nasal bones that form the solid structure of the upper one-third of the nose. Abnormalities in these anatomic structures can also cause obstructed breathing. For example, many people have weak or unfavorably-shaped lower lateral cartilages that cause poor breathing on one or both sides. Likewise, patients with history of nasal bone fracture that was not properly treated may have a deviated bone nasal vault, usually with poor breathing on the side opposite the deviation. Functional rhinoplasty is defined as surgery of the external nasal bones or cartilages for the purpose of improving nasal breathing that is due to any combination of these anatomic deficits. Usually, functional rhinoplasty and septoplasty are considered medically necessary by insurance companies and will be covered procedures when certain specific criteria are met.
